Termination
Either party may terminate the professional relationship with written notice. Upon termination, you remain responsible for payment of fees for services rendered through the termination date.
We will provide access to your applicable records upon reasonable request.
Dispute Resolution
Any disputes arising from our professional relationship will first be addressed through good-faith negotiations. If a resolution cannot be reached, disputes shall be resolved through binding arbitration in accordance with applicable laws in the governing state.
Changes to Terms
